Texas law requires a veterinary referral before rehabilitation care can begin. It is a normal step and your veterinarian will be familiar with it.
If you are the Pet Parent
You do not fill this one in. Send your own veterinarian the link to this page, or print the form and hand it over at your next appointment. They complete it, sign it, and return it to us.
Once it is back with us, call and we will book your first evaluation.
If you are the veterinarian
The declaration confirms that you have established a valid veterinarian / client / patient relationship and examined the patient to determine that these therapies are appropriate.
Fill this in and it goes straight to the clinic. Nothing to print, scan or post. If you would rather use pen and paper, the PDF is right under the form.
Prefer pen and paper? The form can be printed and filled in by hand, then scanned and emailed to info@rehab4pets.com or brought to the first appointment.
Rehab 4 Pets is a rehabilitation practice for small animals. We offer hydrotherapy (underwater treadmill), extracorporeal shockwave therapy, extracorporeal mechano-transduction therapy, laser, chiropractic, customized bracing, exercise therapy and more.
We work alongside the referring veterinarian rather than in place of them. The rehabilitation evaluation is not a diagnostic medical assessment, the medical condition stays with you. We assess function and structure in relation to your diagnosis, and we are glad to share records so the two sides stay joined up.
Per Rule 573.14 of the Texas Administrative Code, the supervising veterinarian will have established a veterinarian / client / patient relationship and determined that rehabilitation services (an alternate therapy) will not likely be harmful to the patient.
Under Rule 573.14 of the Texas Administrative Code, animal chiropractic and musculoskeletal manipulation are classed as alternative therapy. The supervising veterinarian establishes a valid veterinarian, client and patient relationship and determines that rehabilitation services are unlikely to be harmful to the patient.
Your own veterinarian. You send them the link to this page and they can type straight into the PDF and return it by email, or print it and take it to your next appointment. They complete, sign and return it.
Their name, clinic and email, the client’s name and cell number, the patient’s name, the diagnosis including medication profile, pre-existing conditions and diagnostic results, any precautions or contraindications, other medical conditions and medications, and their signed declaration.
Email it to info@rehab4pets.com, or give it back to the client to bring with them. Questions about whether rehabilitation suits a particular patient are welcome, call 303‑388‑6886.
Call us. Whether you are a Pet Parent or a referring veterinarian, we would rather talk it through than have you guess.
