Two forms, and only one of them is yours to fill in. Here is which is which, and the order they happen in.
Two. The veterinary referral form, which your own veterinarian completes and signs, Texas law requires it before rehabilitation care can begin. And the new patient intake form, which you complete yourself with your pet’s history.
No. That one is for your veterinarian. You send them the link, and they can type straight into the PDF and return it by email, or print it and complete it at your next appointment. They sign and return it.
No. Nothing has to be filled in before you call, the forms come once you have decided to go ahead.
First
Your veterinarian fills this in, not you.

Second
This one is yours.
Your pet’s history, what you have noticed, their diet and medications, and the consent agreements. Filling it in before you arrive means more of the first appointment is spent on your pet than on paperwork.
Then you probably don’t. Nothing has to be filled in before you call, the forms come once you have decided to go ahead. If you are still working out whether rehabilitation is right for your pet, start with a phone call instead.
